Parents/Guardians will be asked to complete the Emergency/Registration form for each child. A copy of your information is being provided to your Coach.

Liability Information: All parents, players and coaches should be aware that T-Ball, Softball and Baseball involve an element of risk and/or danger for all participants and may cause serious injury, death or property loss. The Manawa Youth Baseball Softball Association (also referred to as MYSA) DOES NOT provide medical or hospital insurance for participants in our program. All persons participating in MYSA activities must provide their own insurance and assume risk for all injuries. I hereby give consent to receive medical treatment and also agree to indemnify and hold harmless the Manawa School District, Manawa Youth Baseball Softball Association, its Board Members, Coaches, Assistant Coaches and other volunteer parents, from any and all losses, claims, costs, expenses, and/or judgments rising in any manner from the participation or attendance of any MYSA event. Anytime the information on the Emergency Form changes, I will notify the Manawa Youth Baseball Softball Association.

Sportsmanship and Code of Conduct

Conduct – Managers, Coaches, Players, Umpires or Parents who, in the opinion of any member of the MYSA, conduct themselves in an unsportsmanlike manner that would discredit this program may be removed by the Officers/Board of Directors/Coaches from any further participation in MYSA activities.
Social Media – Managers, Coaches, Players, Umpires or Parents who use social media to discuss MYSA, Players or Coaches in a derogatory manner will be removed by the Officers/Board of Directors/Coaches from any further participation in MYSA activities.
Volunteering – By registering your child(ren) in the MYSA summer program, you agree to volunteer a minimum of 2 hours per child, whether in a coaching role or running concessions.
Heckling - No Player, Coach, Manager, Parent or other party associated with any event of the MYSA shall abuse, heckle, or make uncomplimentary remarks to any Coaches, opposing players, team officials or umpires.The umpire shall eject an individual violating this regulation immediately from the game.
Sportsmanship – Any players, coach, manager, parent or other party accompanying a team will be ejected for the remainder of the game from the time of the infraction, if they:
- Use profane language
- Make unnecessary or obscene gestures in protesting an umpire’s decision
- Throw bats, gloves, helmets or any other equipment
- Make unnecessary or obscene gestures to fans or opposing players
- Resort to any unnecessary roughness on the playing field or in the dugout, including but not limited to: hitting, kicking, jumping on or any other physical “inappropriate” contact

We the undersigned, understand the ruling of the unsportsmanlike conduct, and understand that if I/we should act in any disagreeable way with the ruling, I/we may be ejected from the park and may lose the privilege of membership in the Manawa Youth Baseball Softball Association (MYSA). I agree that I will not wear Jewelry of any kind while practicing or playing in a game. I will wear the Uniform required for my sport without alterations.

Concussion Information

As a parent and as an athlete, it is important to recognize the signs, symptoms and behaviors of concussions. By signing this form, you are stating that you understand the importance of recognizing and responding to the signs, symptoms and behaviors of a concussion or head injury.

I have read the Wisconsin Concussion Fact Sheet for Athletes document and understand what a concussion is and how it may be caused.
I understand the importance of reporting a suspected concussion and that it is my responsibility to seek medical treatment
I understand that the student must be removed from practice/play and cannot return until provided written clearance from an appropriate health care provider to his/her coach.
I understand the possible consequences of my child returning to practice/play too soon.
